Are you still administering a health risk assessment the hard way?
Health risk assessments are very useful tools and can provide powerful insights into your employees’ health, behaviors and attitudes. The wellness4one system allows the admin to create custom health
risk assessment questions, combine them into a questionnaire and administer them to the group through the Evaluation tab. Once the assessments are complete, the wellness program administrator can
run reports against them.
The HRA creation process involves two steps: In the first step, the admin creates the questions which can be true / false, any multiple choice or open answered. Once the questions have been written,
they are grouped together into a health risk assessment which is available to all group members on their evaluation tab.

HRA Question Wizard |
The purpose of the HRA Question Wizard is to step a wellness administrator through the process of creating custom Health Risk Assessment Questions for their group.
This function will allow admins to create a new question which will be available for inclusion in the Health Risk Assessment Questionnaire. The questions can be
true or false, multiple choice or open ended (free-form).

The purpose of the HRA Questionnaire Builder is to allow the admin to arrange and organize HRA questions in order to create a questionnaire for users that will be
available on the Evaluation tab.

This report is used to summarize aggregate group data from a health risk assessment created by the administrator. |
