Purpose
The purpose of the Health Risk Assessment (HRA) page is to record user responses from health-related questions assembled by your group administrator. A HRA may be required for group members on a periodic basis or simply available to members to use at their discretion.
Data recorded on the HRA page is used in HRA group reporting.
Features
Once a group admin creates a Health Risk Assessment using the HRA Builder, it is available to all group members. An assessment may include any question created by the admin.
How To
1. After logging into the system, click the Evaluation tab.
2. Select the desired HRA from the Health Risk Assessment drop-down box.
3. Select New from the Assessment Date drop-down box.
4. Click Next on the HRA Wizard title screen.
5. Read the directions and record your results with the provided buttons. When you are ready, click Next.
6. Repeat step 5 until all evaluations are complete.
7. On the calendar provided, click the date when the assessment was performed and then click Finish.
