Purpose
The wellness4one system allows the admin to create custom health risk assessment questions and administer them to the group through the Evaluation tab.
The purpose of the HRA Question Wizard is to step a wellness administrator through the process of creating custom Health Risk Assessment Questions for their group. This function will allow admins to create a new question which will be available for inclusion in the
Health Risk Assessment Questionnaire.
Features
The HRA Question Wizard dialog allows the administrator to create and name a new HRA question, provide a question description, and provide an answer type such as true or false or providing a range.
How To
1. After logging into the system, click on the
Admin tab.
2. Select the desired group and then click the
Add Question link.
3. Click the
Next button on the Introduction step.
4. Provide a name for the question that will be easily identifiable on reports. When you are ready, click
Next
5. Add the question including how it should be answered. Click
Next when you are ready.
6. Set the
Answer Type to either True and False, a Range of choices or Free form entry. If Range is chosen, a textbox is shown in which the user must type the available choices one per line.
7. Click
Finish to complete the wizard.
